Online Driver License and ID Card Application - Required for all driver license and ID card transactions
You must complete this online application for all driver license and ID card transactions. Do not print the application - an associate will access your electronic submission during your appointment.
Certification of Address for Proof of Residential Address
This form must be accompanied by two proofs of Florida residential address in the addressee's name. The proofs must be from different sources. If completed by a shelter or public assistance agency, this form must be accompanied by a letter from the shelter or agency that states they receive mail for the person named therein. Two additional proofs of address are not necessary. A representative of the shelter or agency must sign and indicate the address of the shelter or agency on the form.
